Rates
Level I Basic Room and Board Rate:
$2900 per month
Assisted Living Program:
Additional daily charge based on PRI
Respite (temporary residence for up to 42 days in a calendar year:
$125 per day
An assessment is completed upon admission to identify the needs of each individual and the level of service required.
Services Include:
- 24 hour staffing
- 3 meals per day in central dining room and evening snack; guest meals
- Routine maintenance in resident's room including changing light bulbs, plumbing/heating repairs, and assistance with hanging pictures, etc.
- Daily Activities and transportation to scheduled events in the community
- Case Management Services
- All utilities including cable
- Reminders, cueing, and direction
- Medication management including assistance with re-ordering medication and administration of all prescribed oral medications, inhalers, eye drops, and topical ointments
- Assistance with bathing and grooming
- Assistance with selecting appropriate clothing, and with zippers, buttons, TED stockings, or shoes.
- Scheduling of MD appointments
- Scheduling of transportation to appointments
- Monitoring vital signs and/or blood glucose
- Assistance with order/obtaining medical equipment/supplies
- Housekeeping services to include weekly cleaning of resident's room, daily bed making and trash removal.
- Laundry services weekly to include personal laundry and linens.
